Description
This report proposes the Dubois County Homelessness Coalition (DCHC), a collaborative initiative to prevent and alleviate homelessness through integrated services and public education. The plan emphasizes housing stability, health care access, and mental health support, while leveraging existing resources such as TRI-CAP, public health nurses, and peer counseling. Key components include a referral system, community outreach, and partnerships with local employers to address financial and social determinants of health.
