This book was originally published in 2014 by the Arizona Center for Medieval
and Renaissance Studies at Arizona State University, Tempe Arizona. When the
book went out of print, the press kindly allowed the copyright to revert to the
author, so that this corrected reprint could be made freely available as an Open
