The collection also contains some records of registered nurses and midwives. The records may include some, or all, of the following information: Name; Certificate number; Date certificate was issued by the Board of Medical Registration and Examination; Examination/Court Date: Clerk’s name; Physician’s birthplace and birth date; Medical college attended and degree date; Location; Application certificate; License; Power of Attorney; Affidavit; Letters; and Poll Tax cards.
